Vital Records

The Registrar maintains and provides copies of vital records for the Village of Roslyn.

Death Records:

Records for persons who died in the Village of Roslyn may be requested by the immediate family (spouse, children, parents) or by other persons who can document a legal or medical need. Applicants applying in person should bring their photo identification, proof of relationship to the decedent, proof of need, and the appropriate fee of $10.00 per copy to the Registrar, Incorporated Village of Roslyn, 1200 Old Northern Blvd., Roslyn New York 11576. Applicants will be asked to complete the "Application to Local Registrar for a copy of Death Record" form.

A request for Death Records may be accepted in writing with the appropriate fee under the following conditions:

• The applicant is qualified as stated above and provides the same information requested as when applying in person. 
• The applicant must also provide the name of the deceased and the date and location of death.
